Requesting an Evaluation

If you have concerns regarding your child’s development, and would like to request an evaluation by the district Child Study Team, please follow the procedure outlined below:

Send a signed letter, via US Postal Service or hand delivery, requesting an evaluation by the Child Study Team. Provide details regarding your concerns, the name and birth date of your children, and include any other information that may be of assistance, such as doctor notes / teacher notes / recommendations / health or medical history that is related to your concerns.

Mail or Hand Deliver the letter to:

From the date that your letter is received in our office, per the State of New Jersey guidelines, a Child Study Team Evaluation Planning Meeting will take place within 20 days. You will be contacted by the Child Study Team secretary via mail regarding the date and time of the meeting. You will also receive a copy of the “PRISE” (Parental Rights in Special Education) packet with your meeting notification.
